Transcript Request Form

Class of 2024: Counselors will send your final transcripts to your university by the end of June. Please contact your counselor if you have any questions about your transcript.

Students in the class of 2025 who have questions about college applications and transcripts should contact their counselor. Current students who would like an unofficial transcript may log into myBellarmine and select the Transcripts link in the Performance tile under the Progress tab.

Others who would like to request a transcript should complete this form. Only former students should complete this form. An official transcript must be sent directly from Bellarmine to a third party (e.g. school/scholarship organization/employer). It can be either mailed or emailed. Some recipients will not accept an official transcript via email. Please check prior to requesting your transcript. 

PLEASE NOTE: Only former students should use this form to request a transcript. If you are a third party institution (high school, university, etc) requesting a transcript on behalf of a former student, please request via email and submit a records request or a signed release. If you are the parent of a former student who is still in high school, please send an email to Melinda Michaelson, Registrar.
